HealtheLife Portal

DPH health department clients can use the DPH HealtheLife Portal to access their records online, such as immunizations, test results, and upcoming appointments. You can learn more about HealtheLife by reading frequently asked questions. 

How do I get access?

Following your first visit to a health department, you will receive an invitation via email to register for the portal. Once you have your username and password, download the app to start using HealtheLife.
